Diagnosis

The process of identifying a condition of mental health by its reported symptoms. Is done as part of the evaluation, however continues throughout the duration of treatment, as the diagnosis reflects change, improvement or increase in severity of functioning. The diagnosis contributes to treatment planning and identification of goals to address symptoms of the diagnosis.

Evaluation (Intake or Assessment)

A diagnostic evaluation is a comprehensive assessment of a client's past and current history of emotions, symptoms and responses to challenges in their lifetime. The evaluation is completed for the purpose of identifying areas of challenge and needs for entry into mental health treatment services and effective planning of treatment. A psychiatric evaluation is completed with the same components for the purpose of providing avenues for psychiatric medication in conjunction with mental health treatment services. Evaluations are completed at the onset of treatment to determine diagnosis(es) and treatment needs.

Clients meet with a prescriber who provides psychiatric medication services and reviewsmedicationprogress and/or any challenges with the client/family. Medication management is oftenpart of a comprehensive package of mental health treatment which includes therapy with a clinician.

These skills targetsocial and interpersonal challenges, communication, work related issues, skills for improving mental helath and more.
